If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.

• fill out a Fit note extension form
• phone or visit the surgery from 8am to 6pm to request this.
• If the GP needs to speak to you before issuing a further fit note a member of the Practice team will contact you and advise about this.
• When the GP has issued the fit note this can be collected from reception or e-mailed to the patient.
